Questionnaire design is a science in itself and thus  questionnaires are subject to perpetual improvement. Nevertheless, hopefully the questionnaires will feed your imagination with areas to explore in your own study and provide ideas on how questions may be designed to elicit the specific information you require from patients or other potential respondents.

Questionnaires used in UK Biobank Study

The UK Biobank Study website provides a number of questionnaire used in the associated study involving a range of questions relating to health and diet. Patients’ experiences and levels of satisfaction

Following the development of the White Paper “The new NHS – modern, dependable”, the UK Department of Health has carried out a number of surveys of patients experiences.

National surveys involving primary carecoronary heart disease and cancer patients have been conducted by the Department of Health in England in relation to patient experience of care. The relevant questionnaires can be downloaded by means of the sub-headers below.

The Department of General Pactice and Primary Health Care at the University of Leicester have developed two questionnaires which may be thought of as jointly comprising a patient diary for assessing experiences across the primary-secondary care interface.  The questionnaires were designed to assess satisfaction with the consultation and satisfaction with the surgery separately.

The General Practice Assessment Questionnaire, developed at the National Primary Care Research and Development Centre, University of Manchester is designed to provide patient feedback on the care they receive from their General Practices. Please note the conditions for use provided via a hyperlinked term towards the bottom of the relevant webpage.

Surveys on Student Alcohol Intake

If you are designing a questionnaire on student alcohol intake and are looking for some ideas for questions to include, here are some useful resources to provide you with some inspiration:

relevant publication: Patterns of alcohol drinking in a population of young social drinkers: A comparison of questionnaire and diary measures

It is of interest to observe what issues are covered (such as whether the students live at home) and also how specific one can be about the frequency of drinking and the types of drinks.

Additionally, have a look at the section below. A search for the term ‘alcohol’, for example, at the recommended CASS website will lead you to relevant questionnaires of relevance to the above topics.

 

Survey Question Bank

The Survey Question Bank contains a variety of questions and questionnaires which have been used in valued and recognized surveys.  The topics covered include health related issues (such as mental health and well-being and physical activity and health, to name but a few) and therefore you may find something of relevance there when designing your own questionnaire.   The site is equipped with an excellent search engine enabling you to search all questionnaires simultaneously for specific words or phrases.
