The general principle appears to be that fees are waived for non-funded research projects and all that is required is that a user agreement form be signed and sent to the relevant institute. Note, however, that questionnaires are not directly downloadable from the recommended site.
The Mapi Research Institute has designed an electronic database containing questionnaires which provide instruments (such as the Neuropathic Pain Scale and the Stroke Impact Scale) for measuring health status and quality of life. These are particularly useful in deciding how to design a scale from which patients should choose their individual responses.
